Each area is further divided into two to five individualized areas. These areas require a numerical value (to be typed in) and an optional Suggested Improvement Areas (to be typed into the same box). If you cannot score a specific line-item, type "N/A."

 

Comments you enter into the Suggested Improvement Areas will be summarized with the other reviewers. There will be three staff members reviewing the president. Your review will be read in full by Dave Jezak and Bob Jordan, members of the President's Appraisal subcommittee. Board members may see the specific results, but will, like the president, definitely will see the summarized results. The President will not see the individual results or comments.

 

The Subcommittee will average all of the scores and compare them with the 2005 and 2006 scores. The subcommittee will provide overall Performance Improvement Suggestions and Career Development Suggestions. You are at liberty to recommend any of these also in the final comments box at the end of the form.

 

Honest, candid scores and objective comments allow the organization to improve and will be received in the spirit of continuous improvement.
